messy layout and signup not working

I’ve been using UserPro for 9 years. I got it because I thought it was going to look professional, but for the entire time I’ve used it for my school’s alumni, the display has looked anything but professional. The URL for it is: https://www.awarenessandbodywork.com/members/ There aren’t many settings to fix this, so I just kinda gave up years ago with it and hoped that it would somehow resolve itself with updates. I’d be curious if anyone has a suggestion to make this look like it’s supposed to.

In addition, I have just learned that one is not able to create a profile–when a person hits ‘submit’ it erases the fields and doesn’t go anywhere. It’s possible that it hasn’t worked for 2 years. Wondering if there are any suggestions for troubleshooting this.
