In my case, the extra icons are due to a conflict of UserPro’s CSS and my theme’s bootstrap.css. Both have a class with the same name “icon-ok”.

This is a screenshot I took with Chrome browser’s debugging tool:

I unchecked the background-image from bootstrap.css in the debugging tool, and the issue is gone.

As bootstrap.css is now very popular and used in many WordPress themes, I think it’d be good that UserPro take care of this potential conflict. 🙂

As you deleted the image url from your theme’s CSS, and if your theme needs some of those icons from glyphiconshalflings.png for display, they will no longer showing up. I think it’d be good if UserPro could take care of this CSS class conflict. 🙂
